Roxborough Memorial Offers Free "Mind Your Medications" Cards

Philadelphia, PA-- To assistant members of the community in keeping their personal medication lists current and accessible, Roxborough Memorial Hospital is offering free Mind your Medications cards to all who are interested.

These cards, about the size of a drivers license card, have been created to allow people to carry with them a list of their prescribed and over the counter medications. It is very important for everyone to maintain a record of their medications at all times, said Dr. Loan Vu, director of the Pharmacy at Roxborough Memorial. By keeping these cards up to date and carrying them in their wallets or pocketbooks, the information will be readily available when people come to the hospital for admission or when they visit doctors and other healthcare providers.

Quantities of the free Mind Your Medications cards have been distributed to all physicians on the Roxborough Memorial Hospital medical staff. They are also available at locations throughout the hospital, and by mail by calling 1-866-822-4RMH (764).

Mind Your Medications cards are part of Roxborough Memorial Hospitals continuing efforts to remain in compliance with goal number 8 of the National Patient Safety Goals developed by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO). The goal mandates that hospitals accurately and completely reconcile patient medications across the continuum of care. It also requires that a complete list of a patients medications is to be communicated to the next provider of service when the hospital refers or transfers a patient to another setting, service, practitioner or level of care within or outside the organization.
