REQUIRED HOSPITALIZATION INSURANCE 

All students are required to have hospitalization insurance. Policy information must be submitted to the school prior to the beginning of each school year. It is the student's responsibility to ensure that this insurance remains active during the school year. If there is a change in insurance vendor, please contact the Admissions Office. Please print the following form and submit it to Patti Burke - Director of Recruitment and Admissions and Student Health Nurse.
