Healthcare Requirements for Senior Students
No student will be permitted to come to Orientation (mandatory) Wednesday, July, 2012 without a completed medical form. Completed forms are to be returned to Patti Burke by July 1, 2012.
Year II required documentation:
1. Medical History Form
2. Evidence of health insurance.
3. Drug Testing Clearance - These tests are done the 1st week in June. Patti Burke spoke
to the class and assigned times and dates for this. If you were not at school
during that time, please contact Patti to arrange a time.
Important Note on Injuries/Exposures During Clinical Rotation:
- In the event that an injury or exposure does occur during clinical rotation, it is critical that the student follow the proper procedure. The assigned clinical instructor must be notified immediately. The instructor will then direct the student to the appropriate health care provider according to the hospital policy where the injury and/or exposure occurred, if treatment is necessary. In case of an emergency, the students should be sent to the Emergency Department and not Employee Health.
